  • Annual Enrollment Fee for each family member is non-refundable
  • Annual Enrollment Fees are due at the time of enrollment
  • All Treatment must be Paid IN FULL at each visit to keep the plan in effect
  • The exams, x-rays, and cleaning occur during a the time of enrollment and cannot be carried over to the next year
  • A fee may incur for each broken appointment without 24hr cancellation notice
  • Our program is not transferable to anyone other than the person subscribed to it.
  • Participation cannot be combined with any other dental plan
